Support for SermonSpeaker 5.x is in bugfix mode only, the same as it is for Joomla 3.10.
I will not add any new features to this releases.
Please upgrade to SermonSpeaker 6.x and Joomla 4.x, which is better anyway.

new Sermons not showing up in Backend. I have uploaded them via FTP.

The Dropdown menu is empty. The path is right!

domain.de/predigt/2013/11/sermon.mp3 etc. chmod is 755 for the folders and 644 for files.

It is possible to upload a sermon via the frontend but I'cant select either the ftp one nor the frontend one.

Monthly subdirectories are enabled. I've created the folder "11" for the month also with ftp.

I don't get it.

Which version of SermonSpeaker are you using? Please check the extension manager to see the current version (don't rely on the SermonSpeaker help view).

I know there was a bug in SermonSpeaker 5.1.0 which would get that behavior and which is fixed with 5.1.1.

It should work, I updated my own pages using it. But it is known to be a bit flaky sometimes. I have no clue why it works for some and doesn't for others.
